When coming in to get an FBI or BCI&I web check please bring your driver license or state issued identification and the information needed to fill in the form below. Please do not print off this form and bring it with you. It must be filled out and signed in the presence of the terminal operator. We are completely mobile and can bring our equipment to you. If you have a group of candidates contact us to get more information or to sign up as a vendor and start an account.

We accept cash, check, money order, and all major credit and debit cards. Card transactions will be subject to a $3.00 convenience fee in addition to services purchased.
